Tuberculosis del tracto urinario inferior: presentación de un caso y algunas consideraciones

Abstract

Se presenta un caso de un enfermo con antecedentes familiares de tuberculosis, que ingresó en varias ocasiones en otros hospitales con el diagnóstico de neumopatías inflamatorias. Varios años más tarde presentó aumento de volumen de epidídimos, testículos, cordones espermáticos y próstata (estos dos últimos también dolorosos) y fístula en hemiescroto. Dado que presentaba piuria estéril en orina ácida, se realizaron exámenes para descartar tuberculosis del tracto genitourinario inferior, los cuales fueron positivos. El paciente recibió tratamiento contra esta enfermedad, y se encontraba en ese momento totalmente asintomático. Se concluye que la tuberculosis del tracto urinario inferior puede presentarse de forma aguda y confundirse con infecciones urinarias inespecíficas o de forma subaguda, y con tumores. Su forma de presentación puede ser inusual. Dado el aumento de la prevalencia de esta enfermedad, éste es un diagnóstico a tener siempre en cuenta. La biopsia por aspiración con aguja fina con control ecográfico es una técnica útil en el diagnóstico. Nuestro objetivo fue demostrar la dificultad e importancia del diagnóstico precoz de la tuberculosis genitourinaria del tracto inferior.
