Fibrosis quística: presentación de un caso

Abstract

RESUMEN

Fundamento: la fibrosis quística es una enfermedad congénita y hereditaria cuyas manifestaciones clínicas a nivel de los sistemas respiratorio y digestivo, llevan al paciente a una desnutrición progresiva.
Caso Clínico: paciente masculino, blanco de tres años de edad, acudió al hospital por presentar tos húmeda, vómitos flemosos, y antecedentes patológicos personales de fibrosis quística. Al examen físico en el cuerpo de guardia, se constató murmullo vesicular ligeramente disminuido y ruidos transmitidos por secreciones. Se detectó además ligera deshidratación y no aumento de peso, por lo que se ingresó en el servicio de vías respiratoria del Hospital Pediátrico Universitario Eduardo Agramante Piña de la ciudad de Camagüey. Se le realizó RX de tórax, el cual demostró marcado reforzamiento de la trama pulmonar con microatelectasias y/o infiltrado bronconeumónico bilateral. Se tomó muestra al paciente a través de sonda de Levin, del contenido gástrico para cultivo, y creció Pseudomona aeruginosa variedad mucoide, con sensibilidad a la amikacina, ciprofloxacina y gentamicina. Se le aplicó tratamiento con amikacina, cefotaxima, vitaminas y aerosoles y se fue de alta, mejorado después de una estadía hospitalaria de siete días, con diagnóstico definitivo de fibrosis quística en recaída.
