Broken Heart Syndrome, a diagnostic challenge. Case report
Abstract
Introduction: Takotsubo syndrome is an acute cardiac condition that presents with a clinical picture similar to that of an acute coronary syndrome and is characterized by segmental abnormalities of ventricular contraction, with a normal coronary tree or with non-significant coronary lesions that explain them.
Objective: To describe a case of Takotsubo syndrome in a male patient diagnosed at the Sampaka General Hospital, Malabo, Republic of Equatorial Guinea.
Case presentation: A male patient with a history of diagnosed essential hypertension who was receiving irregular and suboptimal nifedipine treatment presented with typical symptoms of chest pain, profuse sweating and cold extremities. The electrocardiogram revealed myocardial injury currents in the antero- and latero-apical walls, with ST-T segment elevation of more than 2 mm in precordial leads V2 to V6 and a negative T wave in leads I and aVL. An echocardiogram revealed abnormalities in apical contractility with sequential apical dyskinesias, showing remodelling of the apex with a pseudoballoon appearance. Based on the patient's history and the clinical course over 72 hours, this condition was suspected despite the lack of diagnostic resources available to confirm the diagnosis, such as coronary angiography. Reviews continue to reinforce the pillars of treatment (beta-blockers, ARBs, statins, dual antiplatelet therapy, MRA, and nitrates).
Conclusions: The diagnosis of Takotsubo is a challenge because its symptomatology is very similar to acute coronary syndrome and there are no concrete statistics recorded in the Republic of Equatorial Guinea, possibly related to the limitation of diagnostic means such as coronary angiography in health centers, hence the importance of the clinical case presented.
