Fracturas de la fisis proximal de la tibia en el niño

Resumen

Introducción: Las fracturas de la rodilla en edades pediátricas no son lesiones frecuentes, en especial aquellas localizada en la fisis proximal de la tibia.
Objetivo: Actualizar y brindar información sobre las fracturas de fisis proximal de la tibia.
Métodos: La búsqueda y análisis de la información se realizó en un periodo de 92 días (primero de julio al 30 de septiembre de 2023) y se emplearon las siguientes palabras: physeal injuries, physeal injuries and knee, physeal fractures around the knee, proximal tibial physeal fracture. A partir de la información obtenida se realizó una revisión bibliográfica de un total de 143 artículos publicados en las bases de datos PubMed, Hinari, SciELO y Medline, mediante el gestor de búsqueda y administrador de referencias EndNote, de ellos, se utilizaron 35 citas seleccionadas para realizar la revisión, 30 de los últimos cinco años.
Resultados: Se mencionan las características anatómicas más importantes relacionadas con la fisis proximal. Se hace referencia al mecanismo de producción y clasificaciones empleadas en pacientes con esta entidad traumática. Se exponen el diagnóstico clínico e imagenológico así como las modalidades de tratamiento y complicaciones en tempranas y tardías.
Conclusiones: Las fracturas de la fisis proximal de la tibia son lesiones infrecuentes, con alto riesgo de lesión vascular y de síndrome compartimental. Los mecanismos de producción son variados y tienen características específicas en cada caso. Predominan las lesiones Salter Harris tipo II y las modalidades de tratamiento pueden ser conservadoras y quirúrgicas estas últimas con indicaciones muy específicas.
