Microbiological aspects of respiratory colonization by Elizabethkingia meningoseptica in a polytraumatized patient
Abstract
Introduction: Infections caused by Elizabethkingia meningoseptica have a high mortality rate due to the lack of effective therapeutic regimens and its intrinsic resistance to multiple antibiotics. The selection of appropriate empirical therapy at the onset of bacterial infection can be highly challenging due to this resistance.
Objective: To analyze the importance of appropriate management of patients infected or colonized by Elizabethkingia meningoseptica, especially in critical areas. Additionally, the objective is to evaluate the antimicrobial susceptibility profile, considering the differences in antibiotic resistance patterns of this bacteria in different geographic regions.
Case report: The isolation of Elizabethkingia meningoseptica in a polytraumatized patient with resistance to multiple common antibiotics was reported. The identification of the bacteria using the VITEK2® system provided valuable information regarding its antimicrobial susceptibility profile, adding to the challenge of selecting appropriate empirical therapy.
Conclusions: The presence of Elizabethkingia meningoseptica in critical areas’ patients is worrisome due to its high resistance and associated lethality, particularly in cases of bacteremia. The use of vancomycin as a treatment is controversial due to the observed discrepancy between its in vitro performance and clinical effectiveness, warranting further investigation. Moreover, the importance of evaluating the antimicrobial susceptibility profile in each region is emphasized to tailor treatment strategies accordingly.
