Malignant pleural mesothelioma with thrombotic and hemorrhagic phenomena

Abstract

Fundamento: se menciona lo frecuente de los fenómenos trombóticos y hemorrágicos en el mesotelioma pleural maligno, siempre relacionados a la hipercoagulabilidad multifactorial de la sangre propia de las neoplasias malignas.

Objetivo: conocer las causas de los eventos tromboembólicos y hemorrágicos en mesotelioma pleural maligno y su relación con el asbesto.
Desarrollo: se hace un breve recuento de los últimos cuatro casos tratados con este diagnóstico que tuvieron estas complicaciones. Se mencionan la hipercoagulabilidad de la sangre, la tríada de Virchow y los fenómenos paraneoplásicos en los tumores malignos o síndrome de Trousseau. Se señala que no siempre el asbesto está ligado al mesotelioma pleural maligno y que las tres causas principales son: la trombocitosis, la coagulación intravascular diseminada con el consumo de los factores de la coagulación y las plaquetas y la liberación por las células endoteliales del vasoconstrictor endotelina-1.
Conclusiones: se insiste en las causas para comprender el origen de estas complicaciones donde se involucran la trombocitosis, la CID y el vasoconstrictor endotelina-1 del endotelio vascular. Se considera que este tema ha sido poco abordado en la literatura médica nacional.
