Esquema para el diagnóstico de Escherichia Coli Enterohemorrágica y otras categorías enteropatógenas en pacientes con enfermedad diarreica aguda

Abstract

Se propone un esquema para el diagnóstico de cuatro categorías enteropatógenas de E. coli en el que las muestras de heces fecales o hisopajes rectales se siembran en paralelo en los medios agarizados Mac Conkey y Mac Cokey Sorbitol. De los mismos se seleccionan de 3 a 5 colonias (rojas del primero; blancas del segundo). A partir de allí, el esquema propuesto incorpora armónicamente las técnicas propuestas por March, Ratnam y Blanco para E. coli enterohemorrágica O157: H7 con la variante de Barreto para E Coli enterohemorrágicas en general y, aprovechando las particularidades hemolíticas de las categorías enteropatógenas que afectan al humano, utiliza esta variante como punto de partida para su diferenciación y para descartar aquellas cepas carentes de valor enteropatógenos, caracterizadas por una hemólisis total. El tipo enteroinvasivo se diagnostica acorde a lo sugerido por Prats y Llovet, en tanto que las enterotoxigénicas y enteropatógenas clásicas se hacen según Blanco y Blanco. La aplicación de este esquema diagnóstico ha posibilitado el establecimiento de estas categorías enteropatógenas en pacientes con EDA menores de cinco años en investigaciones realizadas en el Centro Provincial de Higiene, Epidemiología y Microbiología de Camagüey.
