Implante de mallas por vía preperitoneal en hernia inguinal recidivante

Abstract

Se realizó un estudio preliminar, explicativo, longitudinal y prospectivo en 70 pacientes operados con el diagnóstico de hernia inguinal recidivante en un período de cinco años (enero 1995–diciembre 1999). Para la aplicación de la técnica quirúrgica se utilizó un equipo de especialistas en Cirugía General perfectamente adiestrado. La técnica quirúrgica empleada incluyó los siguientes elementos: reducción del saco herniario y su tratamiento, reparación clásica por vía posterior con sutura no absorbible a puntos separados desde la reflexión del arco del transverso hasta el tracto ileopúbico o al ligamento de Cooper cuando el orificio herniario así lo requirió, posteriormente se procedió al implante de la malla de Mersillene utilizando la técnica de Rives y se fijó la malla con puntos de anclaje a las estructuras anatómicas mencionadas. La secuencia del seguimiento fue de 15 días, un mes, tres meses, seis meses y al año. Hasta el momento no se han reportado recidivas herniarias en los pacientes a quienes se les aplicó la técnica.
