Heparina no fraccionada y warfarina en la fase aguda del infarto cerebral aterotrombótico

Abstract

Con el propósito de evaluar los resultados del tratamiento con heparina sódica y warfarina en la fase aguda del infarto cerebral aterotrombótico, se diseñó un ensayo clínico controlado, aleatorizado, abierto y de fase II en 60 pacientes que ingresaron con este diagnóstico en la Unidad de Cuidados Intermedios (UCIM) del Hospital Provincial Manuel Ascunce Domenech desde enero hasta diciembre del año 2001. Del total de la muestra 30 pacientes recibieron tratamiento anticoagulante (grupo en estudio) y los 30 restantes recibieron el tratamiento convencional (grupo control). Se encontró un predominio de pacientes con edades comprendidas entre 60 y más años, con antecedentes de hipertensión arterial, tabaquismo, cardiopatía isquémica y diabetes mellitus como los más frecuentes. El grado de discapacidad al año mejoró en el grupo que recibió anticoagulante (70, 1 % de los casos tenían grados I y II según la escala de Rankin modificada) en comparación con el grupo control. La principal complicación en ambos grupos de tratamiento fue la neumonía nosocomial (80 %).
