Ginecomastia por espironolactona. Reporte de un caso

Abstract

El desbalance entre la acción estrogénica y androgénica a nivel de los tejidos de la mama provoca la ginecomastia. Describimos el caso de un paciente de 72 años diagnosticado de hipertensión arterial, el cual fue tratado con dieta hiposódica y espironolactona durante más de diez años y presentó ginecomastia bilateral. Se comenta el papel facilitador de este medicamento, al mismo tiempo que realizamos una breve revisión de los mecanismos fisiopatológicos más importantes que tuvimos en cuenta en el desarrollo de esta afección poco frecuente por el total de drogas que la producen y el número cada vez mayor de medicamentos que provocan este efecto adverso; existen otros procesos capaces de producir esta enfermedad, tanto neoplásicos como hepatopatías crónicas en pacientes de la tercera edad.
