Sensibilidad y especificidad de la biopsia renal en el rechazo agudo

Abstract

Se realizó estudio observacional descriptivo de corte transversal sobre la sensibilidad y especificidad de la biopsia renal, con diagnóstico clínico de rechazo en 69 pacientes transplantados con injerto renal funcionante, en el servicio de nefrología del Hospital Provincial Manuel Ascunce Domenech de Camagüey, desde el 1ro de enero de 1998 al 31 de diciembre de 2002. Se realizaron biopsias renales descritas histológicamente mediante la clasificación de BANFF (1993-1995). Las manifestaciones clínico humorales más frecuentes fueron la elevación de las cifras de creatinina sérica e hipertensión arterial. Se sospechó rechazo en 48 pacientes y se diagnosticó en 39 (81, 3 %), no se sospechó en 21 y se demostró histológicamente en siete pacientes (33, 7 %). El diagnóstico clínico de rechazo tuvo una sensibilidad del 84, 7 % y una especificidad del 60, 8 %. La toxicidad por cidosporina y la necrosis tubular aguda fueron las principales enfermedades imbricadas en el infradiagnóstico del rechazo agudo.
