Hifema traumático

Abstract

El hifema usualmente indica trauma intraocular severo y se presenta en trauma con globo ocular abierto o cerrado. Sus complicaciones incluyen hipertensión intraocular (presión intraocular mayor a 25 mmHg en el 25 % de los casos impregnación hemática de la córnea, formación de sinequias anteriores y posteriores, catarata y una variedad de cambios patológicos relacionados indirectamente. El hifema traumático puede reducir significativamente la visión. Es una enfermedad muy frecuente en niños y en el sexo masculino (traumático). El hifema es una de las lesiones oculares más fáciles de detectar; de acuerdo con la enseñanza tradicional de la evaluación del paciente traumatizado, mediante la búsqueda intencionada de lesiones, al identificar el hifema debe establecerse el manejo, sospechar la existencia de una lesión intraocular grave y vigilar la aparición de hipertensión intraocular. Se clasifica en cuatro categorías, en dependencia de la gravedad de la lesión (cantidad de sangre en cámara anterior), contenido, tiempo de evolución. A pesar de sus complicaciones y secuelas con el avance de la tecnología en Oftalmología, se ha logrado su reabsorción con tratamiento adecuado y oportuno. Por su frecuencia se realizó la presente revisión bibliográfica.
