Manejo hospitalario del trauma craneoencefálico menor en el adulto mayor

Abstract

RESUMEN

Fundamento: el adulto mayor es más vulnerable a sufrir un trauma craneoencefálico. Actualmente el incremento de la población geriátrica a nivel mundial sugiere que el trauma craneoencefálico en los mayores de sesenta años se convertirá en un significativo problema de salud.
Objetivo: describir un protocolo para el tratamiento del trauma craneoencefálico menor en el adulto mayor.
Método: se realizó un estudio descriptivo de ciento veintitrés pacientes con sesenta años y más, que fueron atendidos por el Servicio de Neurocirugía del Hospital Universitario Manuel Ascunce Domenech de Camagüey entre enero de 2004 a enero de 2007, con el diagnóstico de trauma craneoencefálico menor. Se utilizó la clasificación del trauma craneoencefálico menor de Stein - Ross y se aplicaron métodos de estadística descriptiva.
Resultados: se encontró un predominio del sexo masculino y del grupo de sesenta a setenta y nueve años. El 61% de los pacientes padecían alguna enfermedad crónica. El trauma craneoencefálico leve fue la variedad más frecuente y el hematoma subdural crónico fue la complicación de mayor incidencia. No se reportaron fallecidos.
Conclusiones: el trauma craneoencefálico menor en el adulto mayor puede tener una evolución favorable, pero no debe desestimarse y su tratamiento debe estar encaminado al diagnóstico precoz y al tratamiento oportuno de hematomas intracraneales.
