Esclerosis sistémica progresiva y síndrome de Sjögren

Abstract

Se presenta el caso de una paciente de 46 años con antecedentes de haber sido diagnosticada y tratada en este hospital hace tres años de una esclerosis sistémica progresiva difusa. Luego fue ingresada con manifestaciones sistémicas propias de esta enfermedad asociada a un síndrome de Sjögren dado por sequedad de mucosas, (xerostomía y xeroftalmia) y tumefacción parotídea. El estudio de laboratorio incluyendo estudios inmunológicos e histopatológicos de piel y mucosas realizados anteriormente permitió arribar al diagnóstico de esclerosis sistémica progresiva, mientras que el estudio actual incluyendo el oftalmológico (test de Schirmer, Rosa de Bengala, biomicroscopia), gammagrafía parotídea, así como la biopsia labial, demostraron la coexistencia de un síndrome de Sjögren. Igualmente se corroboró que la terapéutica utilizada fue la correcta en esta asociación teniendo en cuenta la respuesta de la misma. Se concluye que la búsqueda sistemática de rasgos compatibles con Síndrome de Sjögren en los pacientes con esclerosis sistémica progresiva debe tratar de resolver en el futuro esta interesante cuestión.
