Primary postpartum hemorrhage in Algerian postpartum women at the first level of care
Abstract
Introduction: Postpartum hemorrhage constitutes a true obstetric emergency in addition to causing deaths, most of them preventable, and it produces a significant number of cases of extreme maternal morbidity.
Objective: To identify the risk factors associated with postpartum hemorrhage in Algerian women who have recently given birth.
Methods: A case-control observational analytical study was carried out, including 21 women with postpartum hemorrhage (cases), and another group of 61 women who did not present this condition (controls), belonging to the polyclinic Charef Wilaya Djelfa, in Algeria, in the period from January to December 2024. Postpartum hemorrhage was the independent variable assessed, and the covariates were risk factors related to uterine tone, trauma, tissue, and thrombin. The odds ratio and its respective 95% confidence interval were calculated for each exposure factor.
Results: Trauma was the main cause of postpartum hemorrhage in the study population in 14 patients (66.7%). There was a significant causal association between multiple pregnancy odd ratio of 4.70, fetal macrosomia and oxytocin (4.58), perineal laceration (4.50), clot retention and episiotomy (4.29), history of postpartum hemorrhage, prolonged labor, and preeclampsia, with odds ratio values of 3.76, respectively, with the occurrence of postpartum hemorrhage.
Conclusions: The risk factors with the highest association were most frequently related to trauma and uterine atony. With the suppression of prolonged labor, the use of oxytocin for induction or acceleration of labor, episiotomies and perineal lacerations, the risk of postpartum hemorrhage was higher.
