Platelet-rich plasma and platelet-rich fibrin combined with arthrocentesis in temporomandibular osteoarthritis

Abstract

Introduction: Osteoarthritis of the temporomandibular joint is a common disease that causes pain and functional limitation. In its treatment, arthrocentesis and intra-articular infiltrations of different substances have been proposed as an accessible, safe and minimally invasive option.
Objective:
To evaluate the efficacy of platelet-rich plasma and injectable platelet-rich fibrin combined with arthrocentesis in the treatment of patients with temporomandibular joint osteoarthritis.
Methods:
A quasi-experimental study was carried out in patients with osteoarthritis of the temporomandibular joint at the Manuel Ascunce Domenech University Hospital and the Martín Chang Puga General Hospital, in the period from September 2023 to September 2024. The study universe consisted of 43 patients and the non-probabilistic sample was made up of 24 patients, who were distributed in two groups of equal quantity. Arthrocentesis was performed in all patients and platelet-rich plasma (Group A) and platelet-rich fibrin (Group B) were subsequently infiltrated. The variables considered in the study were age, sex, pain, and mouth opening.
Results:
The most affected age group was 51 to 60 years old with (41.6 %) n=10 and the female sex with (91.6 %) n=22 of the sample. Patients treated with platelet-rich fibrin showed significant differences in pain relief compared to those who received platelet-rich plasma (p=001). Mouth opening improved in all patients, there were no significant differences between the study groups, however, patients in group B showed better results.
