Characterization of patients with chronic subdural hematoma
Abstract
Introduction: A chronic subdural hematoma develops when the bridging veins rupture and leak blood into the subdural space; its incidence is between 8.2 % and 14 % per 100 000 inhabitants per year.
Objective: To characterize patients with a diagnosis of chronic subdural hematoma treated at the Hospital General Docente Agostinho Neto in the province Guantánamo in the period 2017-2024.
Methods: A descriptive, cross-sectional study was conducted on a universe of 145 patients, a simple random sample of 50 patients was taken. The variables were: age, gender, location, Glasgow scale on admission, complications, symptomatology, time elapsed from the traumatic event to the onset of symptoms, surgical technique, used anesthesia, surgical time, and neurosurgical results.
Results: Female patients predominated in 80 % (40/50) and the majority of patients had a bilateral location of the lesions, representing 24 % (12/50). According to the Markwalder scale, patients were mostly located in grade I, which represented 40 % of them (20/50). Patients reporting a history of trauma preponderated in 80 % (40/50). The most commonly used type of anesthesia was local anesthesia and sedation, which accounted for 80 % (40/50). Neurosurgical complications occurred in only 12 % (6/50) of the patients, which were re-bleeding in 8 % (4/50) of the patients and empyema in only 4 % (2/50) of the patients. 90 % of patients were at level V on the Glasgow outcome scale (45/50).
Conclusions: It can be concluded that in this investigation there was a tendency for this entity to predominate in female patients, fundamentally with a traumatic cause and with an evolution that could extend up to 3 months on occasions.
