Theoretical elements of the enhanced recovery protocol in ocular adnexal oncology surgery
Abstract
Introduction: Ocular adnexal tumors constitute 90 % of neoplasias in Ophthalmology. Eyelid and conjunctival melanomas, as well as sebaceous gland carcinoma, are rare but aggressive, affecting visual function, the integrity of the eyeball, and even the patient's life. The surgical approach to these lesions poses technical and therapeutic challenges for both the treating team and the patients. Postoperative enhanced recovery protocols are emerging as an alternative to optimize perioperative outcomes, reduce adverse events, and shorten convalescence time.
Objective: To characterize the theoretical foundations and current status of enhanced recovery protocols applicable to ocular adnexal oncologic surgery.
Methods: A 60-day systematic review was conducted in the SciELO, CUMED, LILACS, PubMed, and Google Scholar databases, using terms related to the topic. Of 345 identified articles, 30 relevant references were selected for critical analysis, including 21 publications from the last five years.
Results: Although standardized enhanced recovery protocols exist in various surgical specialties, no consensus has been established for their application in ocular oncology. The available evidence supports their benefits, but a dissociation persists between theoretical principles and their clinical implementation in this field.
Conclusions: The adaptation of enhanced recovery protocols to ocular oncologic surgery represents an underexplored area. The absence of specific guidelines underscores the need for research to validate their usefulness in this context. The development of an evidence-based protocol with a multidisciplinary approach could improve patient care in this area.
